Book An Appointment

Appointments can be made from 9am each weekday by phoning 4772253 or by calling in to the surgery. Urgent cases will be seen as soon as possible.If you cannot keep an appointment, please advise us as early as possible as this appointment can be given to another patient.Please advise the receptionist if you require extra appointment time for a special procedure or where more than one patient is required to be seen.Housecalls are reserved for the housebound or the incapacitated. Please do not ask for a housecall if you can attend the surgery. The surgery is better equipped to deal with clinical problems.

Register as a New Patient

You can register by completing a form available from reception. You will need to sign a letter of authorisation to obtain your medical records from your previous GP. If you have a medical card you will be asked to sign a transfer form to have your number transferred to this practice. If you have a medical card please ensure that all information related to your card is valid and in date. There is a 'no accounts' policy and fees must be paid on the day of consultation. The fee charged for the consultation is dependant upon the nature of the service provided. These services may include investigations, injections, review of extra patients, diagnostic tests and treatment procedures.

Request Repeat Prescriptions

Please allow 48 hours for all repeat prescriptions.You can ring the surgery for your repeat prescription or, alternatively, a repeat prescription request form is available at the desk. Please fill out your details and which medications you require. If you do not require some medication for a month or two, please state this on the repeat prescription form, or at the desk when ordering. Please be aware there is a €20 charge for repeat prescriptions.

Get Test Results

It is very important that you contact the surgery for your results. You are advised to ring the Nurses-line Monday- Friday between 2pm and 2.45pm for all your results.

Claim Medical Expenses

You can claim Medical expenses by downloading and filling in the Med 1 form. Alternatively if you have a private medical insurance company, you can contact them directly for their indivivual claim forms,(VHI, Quinn, Aviva).
