Driving Licence

To apply for a driving licence, an eyesight report from your doctor is required. We have the forms here at the surgery or you can download them, and we would be happy to carry out this test for you.

For Over 70's a full medical is also required, again we have these forms at the surgery.

Unfortunatley this service is not covered by Medical or Doctor's Visit cards.
